Job Overview

We are seeking a highly skilled and compassionate Patient Advocate to join our ACT Clinical Team. As a key member of our team, you will work closely with clinicians, clients, and families to develop and implement personalized recovery plans. Your primary responsibility will be to provide guidance, education, and support to help clients achieve sustained recovery.

Key Responsibilities

  • Develop and implement individualized recovery plans in collaboration with clients, clinicians, and multidisciplinary teams.
  • Provide education and training on recovery principles, coping strategies, and relapse prevention techniques.
  • Facilitate group sessions, workshops, and one-on-one coaching to promote client engagement and empowerment.
  • Collaborate with clinicians to ensure comprehensive care and support for clients.
  • Maintain accurate records and documentation of client progress and outcomes.

Requirements

  • Bachelor's degree in a related field or equivalent experience.
  • Two years of experience in a healthcare setting, preferably in a mental health or substance use disorder context.
  • Previous experience as a Peer Support Specialist or similar role.
  • A history of personal recovery from substance use disorder.
  • Certification in Recovery and Wellness Action Planning (WRAP).
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.

Compensation and Benefits

We offer a competitive salary range of $60,000 - $75,000 per annum, depending on experience. In addition, we provide a comprehensive benefits package, including health insurance, paid time off, and opportunities for professional growth and development.
