Recovery Care Coordinator

The Recovery Care Coordinator at The Ark of DC is responsible for coordinating the care of individuals in recovery from addiction. This position requires strong organizational and communication skills, as well as the ability to work effectively in a fast-paced environment.
This role involves developing and implementing individualized care plans, providing coaching and support to patients, and working closely with external stakeholders to ensure the development of program policies and procedures that further the goals and objectives of the agency and the program.
The Recovery Care Coordinator must have excellent customer service skills and be able to maintain a caseload of customers while ensuring they can effectively function within the community. They assure documentation and record-keeping is in compliance with contract, regulatory, and agency requirements and best practices.
This position involves working closely with therapists to ensure high-quality clinical care and developing and implementing individualized consumer treatment or service plans. The Recovery Care Coordinator has the ability to build relationships and rapport with consumers, engage them in recovery-based services, and share knowledge and information to assist consumers in completing phase work packets and other life skills applicable to long-term recovery and independence.
Key Responsibilities:
- Coordinate care for individuals in recovery from addiction.
- Develop and implement individualized care plans.
- Provide coaching and support to patients.
- Work closely with external stakeholders to ensure the development of program policies and procedures.
- Deliver services that may include but are not limited to: crisis intervention, advocacy, medication monitoring, transportation assistance, financial assistance, and referral and linkage to community resources.
- Prepare clinical reports utilizing electronic clinical documentation software.
- Work effectively as a team member, including participating in all required trainings, ensuring the safety of others, and participating in Peer Review/CQI process.
- Oversee diagnostic assessments, development of treatment plans, and continuous monitoring of quality service delivery.
- Provide recovery education and support to persons in recovery for substance use disorder.
- Participate in outreach to community partners to help coordinate services with consumers.
- Provide resource, treatment, and sober support information to individuals interested in recovery.
- Assist in training of new Peer Recovery Support Specialists.
- Demonstrate cultural competency and support development of cultural competency in staff through appropriate trainings and other activities as needed.
- Explain therapist's role and responsibilities and consumer's rights, including confidentiality and mandated reporting.
- Engage families and significant others as needed to support treatment goal. Accept cultural diversity and integrate the family's values and culture into services.
- Apply crisis management skills when necessary to stabilize consumers.
- Ensure that an environment is created, which is supportive, safe, and appropriately structured for individuals seeking treatment and looking to enter recovery.
- Complete monthly reports as required by the contract with DBH as well as the agency and other stakeholders.
Requirements:
- Master's degree in Social Work or Human Services and four years of experience in an alcohol or other drug or mental health setting preferred.
- Combination of comparable skills, experience, and education.
- Understanding and supportive of a trauma-informed system of care.
- Strong interpersonal, engagement, organizational, and time-management skills.
- Excellent written and verbal communication skills.
- Strong computer skills required.
- Ability to support the organization's mission and philosophy and demonstrate sensitivity to cultural diversity and organizational harmony.
- Value for and the ability to deliver excellent consumer services.
- Self-directed and flexible with the ability to meet the needs of the program and the residents.
- Experience and confidence identifying and implementing evidence-based practices.
- Experience working in crisis settings with individuals and families.
- Ability to work flexible hours.
- Program development, evaluation, solid organizational skills, and public speaking.
- Strong written and oral communication skills.
- Commitment to maintain confidentiality.
- Understanding of the Agency's mission and culture and the ability to maintain appropriate boundaries with staff and clients in all circumstances.
Estimated Salary: $60,000 - $80,000 per year.
