Key Responsibilities- Supervise team members to ensure safe, timely, and appropriate services
- Coach team members to problem-solve cases for optimal patient health
- Monitor documentation and data collection for accuracy
- Collaborate with internal and external stakeholders
- Provide oversight for case management activities
- Current Maryland RN or SW license required
- Bachelor's of Science degree required
- Three to five years' experience in Case Management
- Experience with post-acute services beneficial
- Demonstrate skill in clinical case management
- Effective data analysis and critical thinking
- Excellent written and oral communications
- Ability to collaborate effectively with internal and external customers
