Key Responsibilities:- Conduct biopsychosocial assessments and develop individualized plans of care and treatment/discharge plans in collaboration with the interdisciplinary team.
- Provide timely and effective crisis intervention services, utilizing de-escalation strategies and other evidence-based interventions.
- Evaluate patients' social determinants of health and identify opportunities for improvement in quality of care.
- Collaborate with patients, families, and healthcare teams to provide preventive, protective, or supportive services and ensure seamless transitions of care.
- Maintain accurate and timely documentation of patient interactions, assessments, and interventions, adhering to regulatory standards and confidentiality requirements.
- Masters degree in Social Work from a Council on Social Work Education (CSWE)-accredited program.
- Licensed Master Social Worker (LMSW) licensure from the Maryland Board of Social Work Examiners.
- Minimum one (1) year of related experience in social work or case management.
- Experience working in a healthcare setting, preferably in a hospital or medical center.
- Knowledge of relevant medical and legal issues impacting patient care.
- Ability to communicate effectively with diverse populations, including patients, families, and healthcare teams.
