The Case Manager/Residential Services Coordinator plays a crucial role in fostering housing stability and self-sufficiency for clients. The ideal candidate will possess exceptional organizational and interpersonal skills, a dedication to supporting vulnerable populations, and the capability to manage various responsibilities in a dynamic setting.

Key Responsibilities:
  • Case Management:
    • Assess client needs and formulate tailored service plans.
    • Offer continuous support and facilitate access to essential resources.
    • Advocate for client needs within the organization and with external service providers.
  • Residential Services Coordination:
    • Manage daily operations and room assignments at Vendome.
    • Address conflicts and foster community engagement.
    • Ensure adherence to organizational policies and safety standards.
  • Client Support and Advocacy:
    • Provide crisis intervention and cultivate relationships with community partners.
    • Assist clients with housing applications and track their progress.
  • Documentation and Reporting:
    • Maintain comprehensive client records and generate outcome reports.
    • Ensure compliance with grant and funding stipulations.
Qualifications:
  • Bachelor's degree in social work, psychology, human services, or a related discipline.
  • A minimum of 2 years of experience in case management or social services.
  • Strong knowledge of housing resources and local social services.
  • Excellent organizational, communication, and interpersonal abilities.
  • Capability to work effectively with diverse populations.
  • Proficiency in case management software and Microsoft Office Suite.
  • Aptitude for managing multiple tasks in a fast-paced environment.
  • Willingness to work evenings, weekends, and holidays as needed.
  • Physical Demands:
    • Ability to sit, stand, and walk for extended periods.
    • Capacity to lift and carry up to 25 pounds.
    • Ability to respond promptly to emergencies.
