Job Title: Residential Case Manager I

Job Summary:

The Residential Case Manager I is a critical role at Bayview Hunters Point Foundation, responsible for providing comprehensive case management services to residents in our programs. This position involves assessing residents' needs, developing individualized service plans, and connecting them to community resources and services to help them achieve stable housing and self-sufficiency.

Key Responsibilities:

  • Conduct initial assessments and intake interviews with new residents to understand their needs and goals.
  • Develop and implement individualized service plans based on residents' needs and goals, ensuring a holistic approach to addressing their challenges.
  • Provide ongoing case management, including regular check-ins and progress evaluations, to monitor residents' progress and make adjustments to their service plans as needed.
  • Maintain accurate and up-to-date case files and documentation, adhering to Bayview Hunters Point Foundation's policies and procedures.
  • Connect residents with appropriate community resources and services, such as housing, employment, healthcare, and education, and advocate on their behalf to ensure they receive necessary support.
  • Coordinate with external agencies and service providers to facilitate access to resources and services.
  • Offer crisis intervention and support to residents experiencing immediate needs or emergencies, providing a safe and supportive environment.
  • Facilitate support groups, workshops, and life skills training for residents, promoting their personal growth and development.
  • Assist residents with applications for benefits, housing, and other services, ensuring they have access to the resources they need.
  • Work collaboratively with shelter staff, including the House Manager, to ensure coordinated care and support for residents, participating in staff meetings, training sessions, and case conferences.
  • Communicate effectively with residents, staff, and external partners, maintaining a professional and respectful demeanor.
  • Ensure compliance with shelter policies, procedures, and regulatory requirements, maintaining confidentiality and adhering to ethical standards in all aspects of case management.

Requirements:

  • Bachelor's degree in Social Work, Psychology, Human Services, or a related field (preferred).
  • Experience in case management, preferably in a homeless shelter or similar environment.
  • Can demonstrate a commitment to professional development and enhance candidacy.
  • Equivalent work experience acceptable in lieu of degree with a minimum of one (1) year direct experience in case management or related field.
  • Electronic data collection, input, and information maintenance.
  • Clinical supervision toward licensure is available for those on a BBS licensure track if selected.
  • Designing, planning, and implementing program services, with an emphasis on building accountability for program participants as they strive to move forward in their lives.
  • Ensure the health and safety of program participants' and thorough management of the physical plant; this includes safety, hygiene, appearance, and overall aesthetic of the facility.
  • Prepared to respond to emergencies with clear thinking and swift action.
  • Be committed to the mission of Bayview Hunters Point Foundation and possess a good working knowledge of and sensitivity to individuals with substance abuse and/or mental health issues such as PTSD, psychosis, borderline behaviors, bipolar, etc.
  • Certified Case Manager (CCM), Certified Alcohol and Drug Counselor (CADC), or Mental Health First Aid.
  • Bachelors degree in social services, psychology or a related field.

Physical & Additional Requirements:

  • Regular and reliable job attendance.
  • Effective verbal and written communication skills.
  • Proficiency in computer skills, including knowledge of Microsoft Office, Avatar, internet browsers, etc.
  • Exhibit respect and understanding of others to maintain professional relationships.
  • Independent judgment in evaluation options to make sound decisions.
  • Ability to work effectively in an open office environment surrounded by moderate noise and distractions.
  • Frequently required to sit; occasionally walk and stand; travel from the building to other sites.
  • Specific vision abilities required by this job include close vision, distance vision, depth perception, and the ability to adjust focus.
  • The employee must be able to meet deadlines with time constraints.
  • May be asked to occasionally work irregular or extended hours while performing job duties.
  • Valid California's Driver's License (if duties require).
  • Able to meet required state, federal, local and BVHP standards.
  • Live Scan fingerprinting and TB clearance as well as any other medical vaccinations may be required.
  • At least two COVID-19 vaccinations (preferred).
