- Collaborate with care teams, payers, and external entities to ensure seamless transitions and optimal patient outcomes.
- Develop and implement care plans that meet the unique needs of each patient.
- Monitor and evaluate patient progress, making adjustments to care plans as needed.
- Communicate effectively with patients, families, and care teams to ensure clear understanding of patient needs and care plans.
- Stay up-to-date with industry developments and best practices in care coordination and management.
- Nursing diploma (associate's or bachelor's degree in nursing)
- Currently licensed/registered in applicable State
- One (1) year of clinical experience in post-acute care setting preferred
- Prior case management, utilization review, and discharge planning experience preferred
- Certified Case Manager (CCM) or Board Certification in Nursing Case Management preferred
- Excellent communication and interpersonal skills
- Strong organizational and time management skills
- Ability to work independently and as part of a team
- Knowledge of healthcare regulations and laws
- Ability to maintain confidentiality and handle sensitive information
