Behavioral Health Care Coordinator

The Behavioral Health Care Coordinator will assist the Integrated BH team and BH Specialty team with support services to ensure the continuity of care for patients. This role requires a Bachelor's degree in a social science field and a current State of Idaho Social Worker license in good standing. The ideal candidate will have knowledge of case management methods, procedures, and standards for children and adults, as well as strong communication, problem-solving, and interpersonal skills.
Key Responsibilities- Collaborate with the supervisor and care coordination team to review progress toward program goals and adherence to program service models and work flows.
- Collaborate with FHS employees to ensure programs are working together, not duplicating services, and problem-solving any barriers that have been identified.
- Be available to primary care patients and health professionals on a same-day basis and accessible by phone during working hours.
- Manage referrals from FHS staff for patients who need additional resources or services.
- Help the team identify when standard screening and outcome measure instruments are needed and can help administer these if appropriate to the scope of the position.
- Provide support to the PCBH pathway program, including constructing and maintaining registries, preparing evaluation summaries, and providing referrals, patient education, and community resource materials to patients when appropriate and within the scope of the position.
- Contact adults and families who have requested BH case management services following the required process for assessing need, service delivery, and documentation.
- Provide service coordination to FHS behavioral health patients who have been referred to additional services, following up to identify if there are barriers to accessing those services and identifying any resources that may help address those barriers.
- Regular meetings and communication with the supervisor to review and consult on patients involved in case management and service coordination.
- Perform miscellaneous job-related duties as assigned.
