Job Title: Home Health Registered Nurse RN Full Time
Job Summary:
The Home Health Registered Nurse is responsible for providing skilled nursing care to patients in their homes, developing and implementing individualized care plans, and collaborating with healthcare teams to ensure high-quality patient outcomes.
Key Responsibilities:
- Develop and implement individualized care plans for patients, including medical interventions and measurable goals and outcomes.
- Provide education and training to patients and their families on disease management, medication use, and other health-related topics.
- Conduct regular home visits to assess patient needs, provide care, and document progress.
- Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure comprehensive patient care.
- Coordinate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
Requirements:
- Active RN license in the state of application.
- Valid CPR certification.
- 1 year of experience in a healthcare setting, preferably in home health care.
