Job Summary:
Aveanna Healthcare is seeking a skilled and compassionate Registered Nurse to join our team as a Full Time Registered Nurse - Home Health. As a Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, working collaboratively with our interdisciplinary team to develop and implement personalized care plans.
Responsibilities:
- Develop and implement individualized care plans in accordance with physician orders and patient needs.
- Provide skilled nursing care, including wound care, infusions, catheter care, and PICC line dressing changes.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
- Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Document patient progress and care provided in accordance with regulatory requirements and Aveanna Healthcare policies.
Requirements:
- Active RN license in the state of application.
- Valid CPR and driver's license.
- Medicare skilled nursing experience preferred.
- Basic understanding of Oasis.
- 1-year RN experience in a healthcare setting.
