Position Overview

The Registered Nurse – Home Health is responsible for delivering high-quality, patient-centered care in the comfort of the patient's home. This role requires a skilled nurse to develop and implement individualized care plans, educate patients and their families, and coordinate with healthcare teams to ensure seamless care transitions.

Schedule

This full-time position offers a regular Monday to Friday schedule with weekend and weeknight on-call rotations every 6-8 weeks.

Key Responsibilities:

  • Develop and implement comprehensive care plans through collaboration with physicians, patients, and caregivers.
  • Provide education and support to patients and their families to promote optimal health outcomes.
  • Train and orient primary caregivers to ensure the best possible care for each patient.
  • Coordinate with healthcare teams to ensure timely and effective care transitions.
  • Deliver skilled nursing care in patients' homes, utilizing a range of skills including phone triage, patient education, observation/assessment, wound care, infusions, and more.

Requirements:

  • An active RN License in the state of application
  • Valid CPR and Driver's License

Preferred Qualifications:

  • Medicare Skilled Nursing experience
  • Basic understanding of Oasis
  • 1-year RN experience in a healthcare setting
