Aveanna Healthcare LLC is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse RN Full Time. As a Home Health Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, working collaboratively with our interdisciplinary team to develop and implement personalized care plans.
Responsibilities:
- Develop and implement individualized care plans in accordance with physician orders and client needs.
- Provide skilled nursing care, including wound care, infusions, catheter care, and PICC line dressing changes.
- Educate clients and their family members on disease management, medication adherence, and self-care techniques.
- Collaborate with primary caregivers to ensure optimal functioning and provide training as needed.
- Coordinate care with other healthcare professionals, including physicians, therapists, and social workers.
- Document patient progress, including assessments, interventions, and outcomes, in accordance with regulatory requirements.
Requirements:
- Active RN license in the state of application.
- Valid CPR and driver's license.
- Medicare skilled nursing experience preferred.
