Job Title: Home Health Admissions Registered Nurse

Job Summary:

Aveanna Healthcare is seeking a skilled and compassionate Home Health Admissions Registered Nurse to join our team. As a Home Health Admissions Registered Nurse, you will be responsible for providing high-quality, patient-centered care to individuals in their homes, while adhering to confidentiality standards and professional boundaries.

Key Responsibilities:

  • Develop and implement individualized care plans in collaboration with physicians and clients.
  • Provide education and support to clients and their families to promote optimal health outcomes.
  • Coordinate care with other healthcare professionals to ensure seamless transitions and continuity of care.
  • Conduct thorough assessments and evaluations to identify client needs and develop effective care plans.
  • Document all interactions and care provided in a timely and accurate manner.

Requirements:

  • Active RN license in the state of application.
  • Valid CPR certification.

Preferred Qualifications:

  • Medicare Skilled Nursing experience.
  • Basic understanding of Oasis.
  • 1-year RN experience in a healthcare setting.
