Responsibilities:

Your Role Includes:

  • Conducting assessments and creating individualized treatment plans.
  • Regularly evaluating the effectiveness of treatment strategies, participating in team meetings, and family discussions.
  • Completing discharge assessments and making recommendations for home and community safety, along with providing educational resources.
  • Communicating patient needs and progress to the treatment team, physicians, and family members.
  • Collaborating with other healthcare professionals to ensure coordinated care and enhance patient outcomes.

Qualifications:

What You Need:

  • A minimum of a bachelor’s degree in Physical Therapy from an accredited institution.
  • A current, unrestricted license as a Physical Therapist in the practicing state.
  • Basic Life Support or Cardiopulmonary Resuscitation certification as per state regulations.
  • Preferred: At least one year of experience as a Physical Therapist.
  • Knowledge of rehabilitation techniques for complex neurological conditions.
  • Effective and professional communication skills, both verbal and written.
  • Ability to lift up to 50 pounds and assist with patient mobility, which may involve lifting up to 100 pounds while adhering to safety protocols.
  • Physical requirements include fine motor skills, visual acuity, and the ability to stand or walk for extended periods.
  • Additional physical demands may include pushing, pulling, bending, stooping, reaching, kneeling, and positioning frequently.
  • A health screening or examination may be required prior to assignment and periodically thereafter, based on location policies and regulations to ensure physical capability to perform assigned duties with or without reasonable accommodations.
