Key Responsibilities- Conduct thorough patient assessments and identify individualized care needs.
- Evaluate patient progress and outcomes, making adjustments to care plans as necessary.
- Develop and implement effective nursing interventions to promote optimal patient outcomes.
- Collaborate with the healthcare team to coordinate patient care and ensure seamless transitions.
- Provide patient and family education on treatment plans, procedures, and preventative measures.
- Deliver direct patient care that is tailored to the individual's condition and age.
- Participate in Continuous Quality and Performance Improvement initiatives to enhance patient care and outcomes.
- Contribute to the development of departmental budgets and cost-effective services.
- Provide education and training to clinical staff on treatment plans, procedures, and preventative measures.
- Communicate observations and suggestions for improvement to enhance patient care and staff development.
- Current RN licensure in the state.
- Graduate of an accredited wound ostomy care nurse program with current licensure.
- At least 2 years of current experience in the specialty is required.
- Current Basic Cardiac Life Support, Advanced Cardiac Life Support preferred.
- Wound Ostomy Care Nurse (WOCN) certification preferred.
