A Licensed Rehabilitation Specialist with CTW Home Health LLC will conduct assessments and provide therapeutic interventions to assigned clients, assisting them in achieving their highest potential and utilizing learned skills to thrive within their community, respecting their individual capabilities.

Key Responsibilities:

  • Conducts and documents evaluations of assigned clients in line with departmental protocols.
  • Maintains accurate and timely records and reports for all patients.
  • Designs and implements treatment plans aimed at enhancing clients' functional abilities. Utilizes therapeutic exercises and activities to improve or sustain strength, range of motion, and fine motor skills.
  • Teaches clients adaptive techniques and the use of assistive devices for daily living activities, including meal preparation, personal care, and home management.
  • Provides preventive therapy to sustain functional skills; educates clients on energy conservation, joint protection, and safety measures for sensory impairments.
  • Creates home programs and instructs clients or their families on how to implement these plans.
  • Engages in support groups with other team members to foster a collaborative environment.

Education and Experience:

Mandatory:

  • Bachelor's degree in a relevant field
  • Master's degree in rehabilitation therapy
  • Current registration with the relevant professional association
  • State licensure or certification as a Rehabilitation Specialist

Preferred:

  • At least one year of relevant experience

Physical Demands:

  • May require lifting of equipment used in patient care.
  • Potential exposure to infectious materials.
  • Ability to lift up to 50 lbs. as needed.
