Work Schedule: 16 to 24 hours per week with options for Day, Evening, or Weekend shifts.
Location: A mix of office and patient home visits, with occasional remote work flexibility.
Training: In-office training for 3 to 4 weeks.
Responsibilities:
- Coordinate care for assigned clients, creating a goal-oriented care plan from admission to discharge, including ongoing assessments of client needs.
- Conduct supervisory visits to evaluate clients, families, environments, and clinical caregivers, ensuring thorough documentation.
- Oversee the availability and functionality of necessary equipment and supplies for patient care.
- Deliver direct client care as required.
- Manage expectations and satisfaction levels of both internal and external stakeholders.
- Assess the quality and effectiveness of nursing practices, utilizing data to identify collaborative opportunities for service improvement.
- Provide nursing updates and secure re-authorization for continued care.
- Supervise, orient, train, and evaluate clinical field staff.
- Identify and promote professional practice standards within the organization.
- Contribute to the education and professional development of staff and students.
- Participate in employment decisions affecting nursing staff.
- Ensure compliance with company policies, regulations, and professional standards.
- Maintain a professional demeanor in line with registered nurse standards.
- Deliver nursing care based on best practices and ethical principles.
- Participate in after-hours client care as needed.
- Support a culture of quality and safety in client care through adherence to quality assurance plans.
Qualifications:
- Active RN license in the state of practice.
- Minimum of 2 years of hands-on nursing experience within the last 5 years.
- Ability to travel to patients' homes within the designated area.
- Reliable transportation and valid driver's license.
- Current CPR certification from an accredited organization.
- Willingness to supervise nurses providing in-home skilled nursing care.
Physical Requirements:
- Proficient in English communication (speaking, writing, reading).
- Ability to travel; personal transportation is required.
- Occasional lifting and carrying of up to 25 pounds; ability to lift 50 pounds.
- Prolonged periods of walking, standing, bending, kneeling, and reaching.
- Visual and hearing acuity necessary for patient care.
- Ability to respond to emergency situations physically and mentally.
Work Environment:
- Function in diverse environments, potentially exposed to allergens and various conditions.
- Possible exposure to blood, bodily fluids, and infectious diseases.
