You will work closely with our clinical team to ensure that our patients receive the highest level of care and support.
Key Responsibilities:- Oversee the delivery of nursing care to patients in their homes
- Work closely with the clinical team to ensure that patients receive high-quality care and support
- Coordinate training and educational programs for nurses to improve clinical outcomes and competencies
- Assess staff learning needs and develop plans to improve performance
- Function as the first point of contact for field clinicians to assist with patient care escalations
- Coordinate bi-weekly case conferences with the clinical team to discuss patient progress and revise care plans as needed
- Two years' management or supervisory experience in a healthcare setting, preferably home care or primary care clinic
- Graduate of an accredited school of nursing, Bachelor's Degree preferred
- Current CA Registered Nurse License (RN)
- Current Basic Life Support (BLS) Card
- Valid CA Drivers License with acceptable driving record
