About the Role

As a Care Manager Nurse, you will play a vital role in ensuring seamless transitions of care for our patients. We are seeking a highly skilled and compassionate professional to join our team at the Erie Family Health Center.

Key Responsibilities
  • Conduct thorough assessments of patients transitioning between care settings, identifying potential risks and barriers to effective care coordination.
  • Develop and implement personalized care plans, collaborating with healthcare teams to ensure optimal patient outcomes.
  • Provide expert navigation of healthcare systems, facilitating access to necessary resources and services for vulnerable populations.
Requirements
  • Bachelor's degree or 3 years of licensed professional nursing experience required.
  • At least one year of experience working in community health, safety net settings, or with vulnerable populations required.
  • Active CPR card and strong computer skills, including expertise in Microsoft Office and electronic databases.
