Job SummaryWe are seeking an experienced Care Manager to join our team, responsible for providing expert support to individuals transitioning from nursing facilities to community-based settings.
Key Responsibilities- Assess service needs and develop Comprehensive Service Plans to ensure seamless transitions.
- Collaborate with various stakeholders, including healthcare professionals, family members, and caregivers.
- Participate in care plan meetings and case consultations to drive optimal outcomes.
- A Master's Degree in a relevant field, with experience working with diverse populations.
- Daily use of a car, current insurance, valid driver's license, and an acceptable driving record.
- Flexibility to work varied shifts, including weekends, evenings, and holidays.
In return for your expertise and dedication, we offer a competitive salary range of $65,000 - $80,000 per annum, depending on experience, plus a comprehensive benefits package, including 401(k) matching, dental insurance, health insurance, paid time off, vision insurance, and life insurance.
