Salary and Benefits

Job Description

The Healthcare Transition Specialist will work closely with patients, hospitals, and community resources to ensure smooth transitions between care settings. Key responsibilities include:

  • Conducting transition of care outreach to hospitals and patients
  • Completing assessments, care plans, and documentation for patients transitioning between care settings
  • Identifying clinical concerns and acuity, risk factors, and barriers to adherence to discharge plans
  • Providing care coordination/care management services to facilitate transitions between care settings
Requirements

To be successful in this role, you will need:

  • Bachelor's degree or 3 years of licensed professional nursing experience
  • At least one year of experience working in community health, safety net settings, or with vulnerable populations
  • Strong computer skills, including expertise in Microsoft Office and electronic databases

We prefer candidates with bilingual fluency (English/Spanish), experience partnering with community-based organizations, and knowledge of health system navigation and patient education.
