East Bay Innovations is seeking a skilled Case Manager I to join our Transitional Care and Case Management department. As a Case Manager I, you will provide coordination and support to clients and their families in transitioning from nursing facilities or long-term residential facilities to community-based settings. Your goal will be to help clients secure affordable housing, navigate government benefits, and access necessary services such as In-Home Support Services (IHSS) or nursing support.

Key Responsibilities
  • Assist clients in obtaining housing readiness documents, such as ID, Social Security Card, birth certificate, and income verification.
  • Research and apply clients to available housing opportunities.
  • Maintain relationships with landlords, housing agencies, and housing authorities.
  • Accompany clients to housing interviews and meetings.
  • Arrange and transport clients to medical and dental appointments.
  • Provide money management support, including banking and budgeting.
  • Ensure clients have access to free or low-cost transportation in the community.
  • Connect clients with community programs and benefits.
  • Act as a liaison between EBI and other state and local agencies.
  • Maintain accurate and detailed records, including state and program required documentation, case notes, billing information, and client files.
  • Develop comprehensive service plans to ensure ongoing services in the community and support in transitioning from institutional settings to housing.
Requirements
  • Two years of experience working in human services, preferably with programs such as IHSS and Medi-Cal.
  • Bachelor's degree or equivalent experience in the field.
  • Valid California driver's license with a driving record meeting agency standards. Auto insurance and access to a vehicle.
