About the Role

We are seeking a highly skilled and compassionate Field Care Coordinator to join our team at All Ways Caring HomeCare. As a Field Care Coordinator, you will play a vital role in ensuring the highest level of care and support for our clients in their homes.

Key Responsibilities
  • Routinely schedule home visits to meet with clients and their families to assess their needs and provide personalized care and support.
  • Complete all necessary documentation for home visits, including client information and service delivery records.
  • Assist clients with any issues or questions they may have during home visits and provide ongoing communication with clients, families, and care teams.
  • Report any issues, concerns, or problems to the care team and program director.
  • Enter home visit update information into our systems and maintain accurate records of client contact information.
  • Support care coordinators and program directors as needed and participate in external and internal investigations as required.
Requirements
  • High school diploma or equivalent required; two years of supervisory experience or one year of supervisory experience with one year of college preferred.
  • Valid driver's license from the state of residence with a satisfactory driving record and valid vehicle registration and insurance.
  • Excellent customer service and communication skills, with the ability to function well under pressure and manage a large amount of information.
  • Direct care and supervisory experience working with disadvantaged populations, with knowledge and skills in information systems.
  • Ability to assist others and accept direction, with a strong commitment to providing high-quality care and support.
