Key Responsibilities- Routinely schedule home visits to meet with clients and their families to assess their needs and provide personalized care and support.
- Complete all necessary documentation for home visits, including client information and service delivery records.
- Assist clients with any issues or questions they may have during home visits and provide ongoing communication with clients, families, and care teams.
- Report any issues, concerns, or problems to the care team and program director.
- Enter home visit update information into our systems and maintain accurate records of client contact information.
- Support care coordinators and program directors as needed and participate in external and internal investigations as required.
- High school diploma or equivalent required; two years of supervisory experience or one year of supervisory experience with one year of college preferred.
- Valid driver's license from the state of residence with a satisfactory driving record and valid vehicle registration and insurance.
- Excellent customer service and communication skills, with the ability to function well under pressure and manage a large amount of information.
- Direct care and supervisory experience working with disadvantaged populations, with knowledge and skills in information systems.
- Ability to assist others and accept direction, with a strong commitment to providing high-quality care and support.
