Care Coordinator Job Summary

Covenant Health (Tennessee) is seeking a skilled Care Coordinator to join our team at the Perinatal Center. As a Care Coordinator, you will play a critical role in coordinating the care of patients across the continuum of care, facilitating the achievement of quality, clinical, and cost outcomes.

Key Responsibilities:

  • Assess patients' physical, psychological, and learning needs through consultation with healthcare providers and review of medical records.
  • Make recommendations regarding level of care placement based on patients' clinical status, home situation, and insurance benefits/personal resources.
  • Identify opportunities for intervention, including over- or under-utilization of services, lack of education regarding disease process, and lack of an established treatment plan.
  • Assist with patient appointments at necessary facilities prior to delivery.
  • Improve continuity of care between all physicians involved in patient care.
  • Update referring physician offices on current patient status throughout pregnancy and following delivery.
  • Work closely with referring physician offices to ensure all necessary information is transferred back and forth in a timely manner.
  • Connect patients to relevant community resources to increase patient health, awareness, well-being, and satisfaction while reducing the cost of care.
  • Be the primary contact for patients with questions or in need of help.
  • Participate in transdisciplinary team meetings to discuss and plan patient and family needs and discharge planning to the appropriate level of care.
  • Provide critical analysis of situations presenting barriers to achievement of goals.
  • Participate in the development of carepaths, protocols, and outcomes measures.
  • Collaborate with patients, families, and healthcare teams to produce optimum patient outcomes in a cost-effective and efficient manner.
  • Proactively develop and support methodologies that can positively impact patient care outcomes, cost utilization, and staff productivity.
  • Develop clinical knowledge to assist in the development of protocols/pathways/outcomes supporting the continuum of care model in a cost-effective manner.
  • Continually monitor and report trends involving agreed-upon quality care issues.
  • Acquire an understanding of the coding system and work collaboratively with team members to support and improve documentation as it relates to coding more accurately.
  • Conduct utilization review for identified patients within assigned service areas.
  • Collect data concurrently for identifiable outcomes statistics as requested.
  • Evaluate the effectiveness of the Care Coordination model for identified service areas, considering benefits to patients, healthcare teams, and the institution.

Requirements:

  • Minimum of three (3) years of clinical experience working within a women's services center.
  • Current license in good standing with the State of Tennessee (MSSW/LCSW).
