Job Summary

Covenant Health (Tennessee) is seeking a skilled Care Coordinator to join our team at the Fort Sanders Perinatal Center. As a key member of our healthcare team, you will play a vital role in coordinating the care of patients with high-risk pregnancies, ensuring they receive the best possible outcomes.

Responsibilities
  • Assess patients' physical, psychological, and learning needs through consultation with healthcare providers and review of medical records.
  • Recommend level of care placement based on patients' clinical status, home situation, and insurance benefits/personal resources.
  • Collaborate with healthcare teams to identify opportunities for intervention, including over- or under-utilization of services, lack of education, and lack of established treatment plans.
  • Assist with patient appointments at necessary facilities prior to delivery.
  • Improve continuity of care between physicians involved in patient care.
  • Update referring physician offices on current patient status throughout pregnancy and following delivery.
  • Work closely with referring physician offices to ensure necessary information is transferred in a timely manner.
  • Connect patients to relevant community resources to increase patient health, awareness, and satisfaction while reducing costs.
  • Be the primary contact for patients with questions or needs.
  • Participate in transdisciplinary team meetings to discuss and plan patient and family needs and discharge planning.
  • Provide critical analysis of situations presenting barriers to goal achievement.
  • Participate in the development of carepaths, protocols, and outcomes measures.
  • Collaborate with patients, families, and healthcare teams to produce optimal patient outcomes in a cost-effective and efficient manner.
  • Proactively develop and support methodologies positively impacting patient care outcomes, cost utilization, and staff productivity.
  • Develop clinical knowledge to assist in the development of protocols/pathways/outcomes supporting the continuum of care model in a cost-effective manner.
  • Continually monitor and report trends involving agreed-upon quality care issues.
  • Acquire an understanding of the coding system and work collaboratively with team members to support and improve documentation as it relates to coding.
  • Conduct utilization review for identified patients within assigned service areas.
  • Collect data concurrently for identifiable outcomes statistics as requested.
  • Review medical records and discuss with healthcare teams daily to track adherence to specific pathways or plans of care.
  • Promote education and discharge planning with healthcare teams for optimal functional status and independence.
  • Facilitate referrals to the appropriate entity based on identified needs in collaboration with healthcare teams.
  • Continually strive to improve quality care in a cost-efficient manner using creative problem-solving strategies with healthcare team members.
  • Demonstrate effective communication skills and leadership with healthcare team members to facilitate maximum patient outcomes.
  • Consistently evaluate and participate in process improvement initiatives related to increased quality and efficiency, decreased length of stay, and cost reduction.
  • Consistently seek feedback related to the role to enhance the care coordination initiative.
Qualifications

Minimum Education: None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.

Minimum Experience: Must have a minimum of three (3) years of clinical experience working within a women's services center.

Licensure Requirement: Current license in good standing with the State of Tennessee, (MSSW/LCSW)
