Job Title: Licensed Practical Nurse Hospice

We are seeking an experienced Full-Time Weekend Licensed Practical Nurse (LPN) to join our Hospice team in Montgomery, VA.

As a member of our multidisciplinary team, the LPN will be responsible for providing skilled nursing care to our Hospice patients under the direction of a registered nurse and physician's plan of care.

Key Responsibilities:

  • Provides skilled nursing care to clients as directed by the registered nurse and physician's plan of care in accordance with agency policies.
  • Adheres to policies and procedures.
  • Assists in planning and exchanging information with other health professionals to achieve and maintain continuity of client care.
  • Prepares clinical progress notes and follows client care plans as developed by the registered nurse and according to agency policy.
  • Some on-call rotation.

Requirements:

  • Successful completion of a certified licensed practical nursing program.
  • Currently licensed as an LPN in the state of VA.
  • Minimum of one year of clinical experience.
  • Hospice experience strongly preferred.
  • Reliable transportation and company required auto liability insurance.
