Job Title: Family Support Specialist

TeamBuilders Behavioral Health is seeking a highly motivated and compassionate Family Support Specialist to join our team. As a Family Support Specialist, you will play a vital role in providing unconditional care to adults, children, and families in our community.

Job Summary:

The Family Support Specialist will work in partnership with families to conduct intake, assessment, triage, safety planning, care coordination, in-home and community-based therapeutic activities, and plan of care development/monitoring in the Family Services Program.

Key Responsibilities:
  • Form a helping partnership with families and collateral practitioners to accomplish service goals and objectives.
  • Set and maintain boundaries to ensure the safety and protection of family members and maintain a professional helping role.
  • Maintain flexibility in work schedule to meet family's lifestyle and needs, and be available 24/7 for crisis intervention services.
  • Initiate, complete, and update client documentation in accordance with agency and state/federal applicable standards.
  • Work with direct supervisor, clinical supervisor, and treatment team to develop and update Plans of Care to address family needs in targeted life domains as per assessment results.
  • Engage families and conduct in-home therapeutic skills development activities with children, parents, and other family members as identified in the Plan of Care.
  • Recognize and utilize teachable moments and modeling in helping families during their normal routine.
  • Coordinate, conduct, and document monthly team meetings to review family progress and need.
  • Participate in clinical seminars, supervision, CQI meetings, and trainings as assigned.
Requirements:
  • Must be a minimum of 21 years of age or older.
  • Must be bilingual in Spanish and English languages.
  • Bachelor's degree in a human services related field plus 1 year of experience working with SED children; or, Associate degree in a human services related field plus 2 years of experience working with SED children; or, High School Diploma or General Education Development plus 3 years of experience working with SED children.
  • Obtained a Criminal Records Clearance (local, state, and FBI).
  • Maintains certifications in First Aid and CPR.
  • Demonstrated ability to consistently establish positive relationships, and to work effectively and cooperatively with clients, families, colleagues, staff, and community members.
  • Has a thorough knowledge and use of community resources.
  • Must be computer literate in order to effectively maintain compliance with state and federal standards and requirements for use of computerized client assessments, data systems, etc.
