Family Support Specialist

Families and Youth Innovations Plus is seeking a dedicated and compassionate Success Coach to join our team. As a Success Coach, you will play a vital role in supporting families in need by providing guidance, resources, and advocacy to help them overcome challenges and achieve their goals.
Key Responsibilities:- Coordinate services between providers and clients to ensure seamless support.
- Interpret agency services to clients and community, providing clear and concise information.
- Maintain accurate and up-to-date individual case records for clients receiving services.
- Provide specialized case management services to meet the unique needs of each client.
- Educate clients on available community resources and services, empowering them to make informed decisions.
- Develop and implement programs and services to benefit clients and the community.
- Attend community meetings relevant to program and clients, fostering collaboration and partnerships.
- Provide intensive home-based individual and family client services under the CYFD Placement Prevention Services Contract.
- Meet with families at least weekly during the duration of the case, providing regular support and guidance.
- Be accessible to families by phone 24 hours a day, 7 days a week, and provide at least one face-to-face visit per month during non-traditional times.
- Collect, evaluate, and report data in writing to measure the outcomes and effectiveness of CYFD contract activities.
- Provide weekly reports to referral agencies and/or other service providers for mutual clients.
- Participate in case staffing/conferences as needed with referring agencies and/or other service providers.
- Provide parenting education curriculum to families in the home, promoting healthy relationships and parenting practices.
- Bachelor's Degree in Social Work, Education, Counseling, Psychology, Sociology, Criminal Justice, Family Studies, or a related degree approved by CYFD from an accredited college/university.
- Two years of any combination of experience including working with communities on health or social service related matters, social work/case management experience, behavioral health and/or healthcare.
- Level II Success Coach Qualifications: Bachelor's Degree in Social Work from an accredited college/university and four years of any combination of experience including working with communities on health or social service related matters, social work/case management experience, behavioral health and/or healthcare and licensure by the New Mexico Board of Social Work Examiners at the LBSW, LMSW, or LISW level or licensure in another state and qualified to sit for the next testing session.
