Community Health Outreach Coordinator

We are seeking a highly motivated and experienced Community Health Outreach Worker to join our team at Tiburcio Vasquez Health Center. As a key member of our outreach team, you will be responsible for providing education and outreach services to our community, promoting health and wellness, and building relationships with our patients and community members.
Key Responsibilities- Develop and implement outreach and education programs to promote health and wellness in our community.
- Provide education and support to patients and community members on health-related topics, including COVID-19 prevention and vaccination.
- Collaborate with other departments and community-based agencies to achieve program goals and objectives.
- Participate in community events, health fairs, and other outreach activities to promote our services and build relationships with our community.
- Develop and maintain relationships with community leaders, organizations, and stakeholders to promote our services and build partnerships.
- Assist in tracking data and evaluating program outcomes to ensure we are meeting our goals and objectives.
- Bilingual in Spanish and English required.
- High school certificate (or G.E.D) required.
- BLS certification required (must be obtained prior to start date).
- Minimum one-year experience working within a health care setting, preferably non-profit.
- Previous experience with group training and/or facilitating group meetings preferred.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
- Strong organizational and time management skills.
- Ability to work independently and collaboratively as part of a team.
