Job Summary

Tiburcio Vasquez Health Center is a non-profit community health center dedicated to promoting the health and well-being of our community. We are seeking a skilled RN Care Manager to join our Street Health Outreach team.

Job Description

The RN Care Manager will provide nursing services, including health assessments, medical care, and guidance, to individuals experiencing homelessness and chronic health and mental health needs. This role will work closely with the Street Health Program Manager and medical providers to develop and implement patient care plans.

Responsibilities
  • Conduct health assessments and provide medical care, including vaccinations and wound care, to patients in unhoused encampments and other service locations.
  • Develop and implement patient care plans, including referrals to other providers and community health resources.
  • Provide health education and support to patients, including information about health and community services.
  • Work collaboratively with the Street Health Program Manager and medical providers to ensure high-quality patient care.
  • Supervise the Community Health Worker and Behavioral Health Case Manager.
  • Participate in regular meetings with Alameda County Healthcare for the Homeless.
  • Provide culturally competent, trauma-informed, and harm reduction-based services.
Requirements
  • Current, valid California RN license.
  • Current, Valid Cardio Pulmonary Resuscitation Card (CPR).
  • Valid CA driver's license and insurance.
  • Preferably, two years of nursing experience serving patients with complex needs and ideally, in unsheltered populations.
Qualifications
  • Bilingual in English, Spanish, or other languages is preferred.
  • Ability to work independently, organize tasks, problem-solve, make decisions, work collaboratively as a team member, and handle multiple priorities.
  • Demonstrated experience with quality improvement processes to improve patient care and meet required health risk outcome measures.
  • Culturally sensitive and demonstrated ability and effectiveness working with ethnically diverse populations.
  • Sensitivity to and understanding of the special needs of unsheltered people facing complex challenges.
  • Ability to effectively intervene in crisis situations with de-escalation techniques.
  • Desire to outreach to unsheltered community members.
  • Excellent communication and documentation skills at level necessary for taking patients' medical histories, and accurately documenting patients' medical information.
  • Ability to effectively communicate with patient population and staff while demonstrating a high degree of diplomacy and tact.
  • Ability to multi-task and work effectively in a high-stress and fast-moving environment.
  • Possess a thorough understanding of the importance of confidentiality and non-disclosure according to the general standards set forth by HIPAA.
