KEY RESPONSIBILITIES:

  • Evaluates patients' understanding and educates them on the connection between diet and kidney health, as well as its implications for treatment.
  • Provides guidance to patients, families, and caregivers regarding nutritional status, adequacy measures, fluid management, and all nutrition-related laboratory results relevant to their condition.
  • Conducts comprehensive nutritional assessments and collaborates with the interdisciplinary team to create personalized care plans for patients.
  • Maintains thorough and accurate medical documentation, including progress notes.
  • Offers continuous assessment and counseling to help patients achieve and maintain optimal nutritional health.
  • Identifies patients at risk of malnutrition and collaborates with the interdisciplinary team to determine suitable interventions and resources.
  • Monitors, adjusts, and documents responses to nutritional therapies and addresses any challenges affecting patients' ability to meet their goals.
  • Works with the interdisciplinary team on Urea Kinetic Modeling recommendations to fulfill the physician's Kt/V prescription.
  • Participates in nutrition Quality Assurance and Improvement (QAI) initiatives and reports on nutrition-related outcomes.
  • Reviews nutrition-related laboratory results and counsels patients and/or caregivers to formulate appropriate actions.
  • Recommends treatment modifications to the interdisciplinary team as necessary.
  • Identifies candidates for nutritional supplement referrals to physicians.
  • Communicates effectively with physicians and facility staff as needed.
  • Maintains dietetic registration and continuing education hours as mandated by the American Dietetic Association and applicable state licensure regulations.

EDUCATIONAL REQUIREMENTS:

  • Registered Dietitian credentialed by the Commission on Dietetic Registration.
  • Board Certification as a Specialist in Renal Nutrition is encouraged.
  • Current state licensure, if applicable.

EXPERIENCE AND QUALIFICATIONS:

  • Minimum of 1 year of experience in clinical nutrition as a Registered Dietitian.
  • Previous experience in renal nutrition is preferred.
