**Responsibilities:**
* Administer skilled nursing care to patients requiring professional nursing service per physician's orders and under the supervision of the Supervisor of Clinical Services.
* Manage each patient's care efficiently and effectively to achieve positive patient outcomes.
* Develop and implement individualized care plans, taking into account patient needs, preferences, and goals.
* Collaborate with other healthcare professionals, including LPNs and HHAs, to ensure seamless care delivery.
* Educate patients and their families on disease management, medication adherence, and self-care techniques.
* Conduct regular assessments and evaluations to monitor patient progress and adjust care plans as needed.
* Maintain accurate and detailed records of patient interactions, treatments, and outcomes.
* Participate in interdisciplinary team meetings to discuss patient care and address any concerns.
**Requirements:**
* Current Registered Professional Nurse license.
* 2+ years of acute care nursing experience and/or previous home care nursing experience preferred.
* OASIS experience strongly preferred.
* Med-surge and wound care experience preferred.
* Associate's Degree from an accredited college required; Bachelor's degree preferred.
