Substance Abuse Treatment Coordinator

Options for Community Living is a leading provider of community-based services for individuals with mental illness and substance abuse issues. We are seeking a highly skilled Substance Abuse Treatment Coordinator to join our team.

About the Role

The Substance Abuse Treatment Coordinator will provide support services to individuals with substance abuse issues, working closely with a multidisciplinary team to develop and implement personalized treatment plans. This role will involve conducting routine assessments of residents' substance use needs, providing addiction education and counseling, and connecting residents with community resources and support services.

Responsibilities

  • Conduct Routine Assessments

Perform routine assessments of residents' substance use needs, identifying areas for improvement and developing strategies for addressing these needs.

Provide Addiction Education and Counseling

Provide individual and group counseling to residents, focusing on addiction education and related topics. This may include teaching residents about the risks and consequences of substance abuse, as well as strategies for maintaining sobriety.

Connect Residents with Community Resources

Assist residents in accessing and utilizing community resources, such as D.S.S., medical clinics, and sponsorship programs.

Document Progress and Develop Service Plans

Document residents' progress and develop individualized service plans, including discharge plans as necessary.

Communicate with Leadership and Coworkers

Communicate pertinent information regarding residents to leadership staff and coworkers, enhancing the quality of care provided to residents.

Requirements and Qualifications

  • High School Diploma or Equivalent

A high school diploma or equivalent is required, with a four-year college degree in a human services-related discipline preferred.

Valid CASAC Certification

A valid CASAC certification is required, with at least two years' experience working with individuals with substance abuse issues.

Case Management Experience

Two years' experience working with case management responsibilities in a setting with disabled or economically disadvantaged individuals/families is preferred.

Valid Driver's License and Vehicle

A valid driver's license, safe driving record, access to a vehicle, and valid auto insurance are required.

Location and Schedule

This role is based in our Ronkonkoma and Hempstead offices, with field visits required in both Nassau and Suffolk County. The schedule is Monday - Friday, 37.5 hours/week, with a non-exempt pay type.

Salary Range

The estimated salary for this role is $46,800 - $58,500 per year, based on experience and qualifications.
