Job Summary:

Options for Community Living is seeking a skilled Care Coordinator to provide comprehensive case management services to clients with chronic conditions. The ideal candidate will have a strong background in healthcare and human services, with experience in case management and coordination of care.

Key Responsibilities:
  • Provide case management services to clients, including assessment, goal setting, and coordination of care.
  • Develop and implement individualized service plans to meet the unique needs of each client.
  • Collaborate with healthcare providers, family members, and other stakeholders to ensure seamless care coordination.
  • Monitor client progress and make adjustments to service plans as needed.
  • Ensure compliance with Health Home standards and regulations.

Requirements:
  • Bachelor's degree in healthcare or human services.
  • At least two years of experience in case management or a related field.
  • Valid driver's license and access to a vehicle.
  • Bilingual preferred (English/Spanish speaking).

Work Environment:
  • In-person based out of our Ronkonkoma office.
  • Field visits required within Suffolk and Eastern Nassau County.
  • 37.5 hours/week, Monday - Friday, 8:00 AM - 4:00 PM, 8:30 AM - 4:30 PM, or 9:00 AM - 5:00 PM.
