Case Manager, Adult Mobile Crisis

At UMass Memorial Health, we are committed to providing exceptional patient care and academic excellence. We are seeking a skilled Case Manager, Adult Mobile Crisis to join our team.
Job Summary:The Case Manager, Adult Mobile Crisis will be responsible for implementing program policies and procedures to maintain daily structure and consistency within the program. This includes assessing clients' basic physical, social, educational, legal, and service needs and coordinating care with other inter-agency and community-based providers.
Key Responsibilities:- Develop and implement comprehensive treatment plans in collaboration with the multidisciplinary treatment team.
- Conduct outreach and create pathways for referrals, working closely with other providers to receive referrals and conduct admissions assessments.
- Assist clients in engaging next-level services and coordinate transportation as needed.
- Participate in multidisciplinary team meetings to discuss client progress and make referrals as necessary.
- Lead skill development, educational, or recreational group activities as assigned.
- Document treatment provided in the Electronic Health Record (EHR), assessments, follow-up interviews, case conferences, behavioral issues, coordination with other agencies or programs, referrals, clinical team treatment decisions, discharges, and others.
- Bachelor's degree in psychology or human services or a closely related field.
- Satisfactory CORI background check.
- Excellent oral communication and computer skills.
- Ability to multitask and prioritize tasks effectively.
- Requires personal and professional accountability, self-management, and initiative.
- Prior experience in a behavioral health or social services setting.
- Basic knowledge of CHL services and insurance requirements and reimbursement for these services.
- Working knowledge of Electronic Health Records (EHR).
This position requires travel from the base location to other facilities and patient residences, with primary work being indoors in both patient care facilities and alternative patient care environments.
