Families Together of Orange County (FTOC) is seeking a highly motivated and detail-oriented Patient Outreach and Enrollment Coordinator to join our team. As a key member of our outreach and enrollment team, you will play a critical role in increasing access to quality healthcare for individuals and families residing in Orange County.
Key Responsibilities- Identify and Engage Eligible Clients: Conduct direct outreach activities and collaborate with community partners, churches, schools, and internal data to identify individuals who qualify for FTOC services and programs.
- Assist with Enrollment Applications: Provide guidance and support to eligible clients in applying for publically sponsored health care insurance plans through Covered California (Calheers) or BenefitsCal, ensuring timely and accurate enrollment.
- Track and Follow-up on Applications: Maintain a tracking system to ensure all aspects of the enrollment process are complete, from application to post-enrollment, and that clients receive the necessary services.
- Provide Excellent Customer Service: Offer exceptional support to patients with questions or concerns about health coverage and other social service programs, ensuring a positive experience.
- Collaborate with Social Services Agencies: Work with local agencies to troubleshoot application issues and provide additional support to patients as needed.
- Maintain Confidentiality and Accurate Records: Safeguard patient information and maintain accurate records, adhering to confidentiality standards and regulations.
- Participate in Community Outreach and Education: Engage with patients and community members to raise awareness about the availability and importance of primary care providers and health plans, promoting access to quality healthcare.
- Support Further Social Service Needs: Connect patients with local agencies and resources to address additional social service needs, ensuring comprehensive support.
- High School Diploma and 1 year of experience in eligibility and verification of healthcare benefits, specifically Medi-Cal or CalOptima Health.
- CoveredCA Certified Counselor certification is highly preferred.
- Verbal Communication, Organization, Scheduling, Professionalism, Customer Focus, Confidentiality, Bedside Manner, Medical Teamwork skills are essential.
- Bilingual (Spanish) and a valid CA Driver License are required.
