Job Summary

Bridgewell is seeking a skilled Licensed Practical Nurse (LPN) to join our team in a residential program. As an LPN, you will provide high-quality health services and direct care to individuals with disabilities, working collaboratively with our multidisciplinary team to ensure the best possible outcomes.

Key Responsibilities
  1. Provide direct nursing care to individuals with disabilities, including medication administration, wound care, and vital sign monitoring.
  2. Document health care services and maintain accurate medical records, adhering to state regulations and nursing standards.
  3. Collaborate with the residential manager, registered nurse, and Director of Health Care to ensure seamless care coordination.
  4. Act as a liaison with community health care providers, monitoring medical records and advocating for individuals' needs.
  5. Provide in-service education to individuals and staff on health issues, promoting a culture of health and wellness.
Requirements
  • Graduation from an accredited LPN program and current Massachusetts Nursing license.
  • Two years of general nursing experience, preferably in a community health setting or with individuals with developmental disabilities.
  • Excellent communication and organizational skills, with the ability to foster positive relationships and work collaboratively with the team.
  • Basic to intermediate computer level competency and ability to maintain accurate medical records.
