Licensed Practical Nurse

Bridgewell is a leading provider of social and human services that empower individuals with disabilities and other life challenges to live safe, self-directed, and productive lives. We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team in a residential program.
Job Summary:The LPN will provide health services and direct care to individuals with disabilities living in a residential program. This includes providing specialized nursing care, monitoring medical records, and administering medications. The successful candidate will work collaboratively with the residential manager, registered nurse, and Director of Health Care to ensure the highest level of care for our residents.
Key Responsibilities:- Provide direct specialized nursing care to individuals with disabilities
- Monitor medical records and administer medications as needed
- Collaborate with the residential manager, registered nurse, and Director of Health Care to ensure high-quality care
- Document health care services and maintain accurate medical records
- Provide in-service education to individuals and staff on health issues
- Maintain knowledge of Department of Public Health Medication Administration Program (MAP) policies and procedures
- Graduation from an accredited and approved licensed practical nurse program
- Current Massachusetts Nursing license
- Two years of general nursing experience and/or service to individuals with developmental disabilities in a community health setting
- Excellent written and oral communication skills
- Ability to foster positive relationships
- Basic to intermediate computer-level competency
