Job Title: Licensed Practical Nurse

Bridgewell is seeking a skilled Licensed Practical Nurse (LPN) to join our team in a residential program. As a LPN, you will provide health services and direct care to individuals with disabilities, working collaboratively with our residential manager and registered nurse to ensure the highest level of care.

Responsibilities:
  • Provide direct specialized nursing and other day-to-day care to individuals
  • Document health care services following appropriate state regulations, nursing standards, and Massachusetts LPN license requirements
  • Act as a liaison to other community agencies and treatment providers regarding regular medical needs and care
  • Provide in-service education to individuals and/or staff on health issues
  • Administer medications to assigned individuals following MAP policies and procedures
  • Provide quality assurance reviews of medication systems and MAP compliance
  • Work collaboratively with the residential manager and registered nurse to assess, organize, update, and maintain health care and medication needs
Requirements:
  • Graduation from an accredited and approved licensed practical nurse program
  • Current Massachusetts Nursing license
  • Two years of general nursing experience and/or service to individuals with developmental disabilities in a community health setting is preferred
  • Experience working with adults with DD and behaviors is a plus
  • Must have a Driver's license for at least one year with a good driving record history
  • Must be able to drive a 12-passenger van
