Position Overview

SUMMARY

The Clinical Social Worker is responsible for delivering and coordinating a comprehensive array of psychosocial services tailored to participants and their families. This role also encompasses community-oriented case management services for program participants.

KEY RESPONSIBILITIES
· Engage in social work by developing and sustaining a professional rapport with participants.
· Act as a liaison and resource for all PACE Centers, ensuring that all social workers fulfill their responsibilities efficiently and effectively.
· Strategically plan, organize, and oversee the workflow for assigned social workers within the program.
· Maintain accurate records and generate reports as required.
· Participate in the Interdisciplinary Team (IDT) initial assessments to identify the services necessary for each participant.
· Recognize warning signs and other issues, establishing measurable goals.
· Guide participant needs and facilitate IDT meetings as outlined in the Plan of Care.
· Evaluate the quality and effectiveness of services delivered to participants, adhering to individualized care plans.
· Document all participant interactions promptly, in compliance with program standards and policies.
· Update and enter all required participant information.
· Refer participants to suitable community agencies or facilities, advocating on their behalf.
· Attend and contribute to IDT meetings, enhancing understanding of psychosocial care aspects and providing the team with clearly defined psychosocial objectives.
· Collaborate with other professionals to ensure quality and continuity of care for participants.
· Show respect for colleagues and IDT leadership.
· Employ creative critical thinking to address patient and family concerns.
· Fulfill additional duties and responsibilities as assigned.
· Participate in staff meetings, in-service training, continuing education, and workshops.
· Engage in Quality Assessment and Performance Improvement initiatives as directed.
· Ensure timely completion of assessments.
· Initiate appropriate psychosocial case management in accordance with established policies.

Maintain documentation of assessments and interventions that reflects thoroughness and timeliness.
· Assist participants with care and transportation as needed.
· Execute other tasks as delegated by management.

SUPERVISORY ROLE
No supervisory responsibilities.

QUALIFICATIONS
(EDUCATION and/or EXPERIENCE)
· A Master's degree or higher in Social Work (CSWE accredited) is required.
· 3-5 years of experience in a case management role, with at least 1 year of experience working with elderly and frail populations.
· Proven ability to network with community resources and a strong understanding of available services.
· Excellent communication skills, encompassing face-to-face interactions, telephone communication, and computer proficiency.
· Strong attention to detail and organizational skills.
· Ability to interact positively with participants, families, and external agencies, presenting a professional image.
· Capacity to manage multiple priorities effectively while handling the emotional demands of the role.
· Bilingual candidates are preferred.
· Ability to work independently, exercising discretion and sound judgment.
· Physical capability to access various environments, including homes, hospitals, nursing facilities, and assisted living facilities.
· Willingness to travel within the community.
· Exceptional telephone communication skills.
· Must possess a valid driver's license, automobile insurance, and reliable transportation.

CERTIFICATIONS, LICENSES, and REGISTRATIONS
Florida Driver's License and Auto Insurance MSW

WORK SCHEDULE
Full-time position with an 8-hour shift.
