POSITION SUMMARY
The Social Worker is responsible for delivering and coordinating a comprehensive array of psychosocial services tailored to participants and their families. This role includes providing community-oriented case management services to program participants.
KEY RESPONSIBILITIES
· Engage in social work by developing and sustaining effective relationships with participants.
· Act as a resource and liaison for all service centers to ensure social workers fulfill their responsibilities efficiently.
· Organize and oversee the workflow for assigned social workers within the program.
· Maintain accurate records and generate necessary reports.
· Participate in interdisciplinary team assessments to identify participant needs.
· Recognize warning signs and challenges, establishing measurable goals.
· Guide participant needs and coordinate visits with interdisciplinary team members as outlined in the care plan.
· Evaluate the quality and effectiveness of services delivered according to the personalized care plan.
· Document participant information promptly, adhering to program standards and policies.
· Update and manage all required participant information.
· Connect participants with suitable community resources and advocate on their behalf.
· Attend and contribute to interdisciplinary team meetings, enhancing understanding of psychosocial care aspects and providing the team with clearly defined psychosocial objectives.
· Collaborate with other professionals to ensure high-quality and continuous care for participants.
· Show respect for colleagues and leadership within the interdisciplinary team.
· Employ creative problem-solving skills to address patient and family issues.
· Carry out additional duties as required.
· Participate in staff meetings, training sessions, and professional development opportunities.
· Engage in quality assessment and performance improvement initiatives as directed.
· Ensure timely completion of assessments.
· Initiate appropriate psychosocial case management in line with established policies.
DOCUMENTATION AND ADMINISTRATIVE DUTIES
Maintain thorough and timely documentation of assessments and interventions.
· Assist participants with care and transportation as needed.
· Execute other responsibilities as assigned by management.
SUPERVISORY ROLE
No supervisory responsibilities.
QUALIFICATIONS
EDUCATION and EXPERIENCE
· A Master's degree or higher in Social Work from an accredited program is required.
· 3-5 years of experience in case management, with at least 1 year focused on the elderly and frail population.
· Proven ability to network with community resources and a strong understanding of available services.
· Excellent communication skills, both verbal and written, including proficiency in technology.
· Strong organizational skills and attention to detail.
· Ability to interact positively with participants, families, and external agencies, presenting a professional image.
· Capacity to manage multiple tasks and effectively cope with the emotional demands of the role.
· Bilingual candidates are preferred.
· Ability to work independently while exercising sound judgment and discretion.
· Physical capability to access various environments, including homes and healthcare facilities.
· Willingness to travel within the community.
· Strong telephone communication skills.
· A valid driver's license, automobile insurance, and reliable transportation are required.
CERTIFICATIONS
Must possess a valid driver's license and auto insurance.
WORK SCHEDULE
This position requires in-person attendance during standard working hours.
