Key Responsibilities:

  • Participate in patient care activities in the clinic with the provider and team on designated days.
  • Collaborate with local organizations, such as the Alzheimer's Association, Area Agency on Aging, and Martin County SHIP, to facilitate community engagement and outreach for memory and movement disorders.
  • Administer the Montreal Cognitive Assessment (MOCA) test, with the opportunity to obtain certification upon onboarding.
  • Maintain State Contract Requirements and ensure compliance with federal, state, and local regulations.
  • Develop and maintain knowledge of Alzheimer's disease, memory disorders, and movement disorders, and provide initial and follow-up psychosocial and functional assessments.
  • Refer patients to community resources, arrange appointments, and establish rapport with other agencies to ensure comprehensive care.
  • Provide referrals and counseling as needed, and maintain accurate records of patient cases.
  • Work closely with physicians to ensure patient compliance with treatment plans and maintain confidentiality of patient information.
  • Contribute to team efforts by accomplishing related tasks and maintaining operations by following policies and procedures.

Requirements:

  • Bachelor's Degree in Social Work required.
  • Licensed Clinical Social Worker preferred.
  • At least one year of medical experience preferred.
  • Ability to lead and coordinate activities of a diverse group of people in a fast-paced environment, with critical thinking and problem-solving skills and computer literacy.
