Hospice Nurse Case Manager Opportunity

We are seeking a dedicated Hospice Nurse Case Manager to join our team at Resource 1 Homecare Staffing. This rewarding role offers the opportunity to make a meaningful impact on patients and their families during their end-of-life journey.
About the RoleThe successful candidate will be responsible for delivering high-quality patient care, ensuring that each individual receives personalized attention and support. With a focus on community health education and experience, our ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work efficiently with minimal supervision.
Responsibilities- Conduct comprehensive assessments of patients and their families to identify needs and develop effective care plans.
- Provide professional nursing care, utilizing all elements of the nursing process to ensure optimal outcomes.
- Collaborate with the interdisciplinary team to coordinate care, promoting continuity and seamless transitions.
- Document patient information accurately and thoroughly, maintaining up-to-date records and statistics.
- Participate in quality assessment performance improvement teams and activities, contributing to the enhancement of our services.
- A valid driver's license and reliable vehicle for travel to patient homes and facilities.
- Graduation from an accredited school of nursing, with a strong background in hospice care or a related field.
- 1-2 years of recent acute care experience, preferably in an institutional setting, or 1-2 years of home care experience.
- A passion for hospice care and the ability to build relationships with patients and their families.
- An attractive salary of $75,000 - $85,000 per year, with opportunities for professional growth and development.
