Job SummaryWe are seeking an experienced Registered Nurse (RN) to join our team as a Senior Hospice Care Nurse Specialist. This role involves covering nursing needs across our territory, providing comprehensive assessments, developing plans of care, and collaborating with interdisciplinary teams to ensure exceptional patient and family satisfaction.
Responsibilities- Patient and Family Assessments/Case Management:
- Conduct thorough assessments of patients' physical, emotional, spiritual, and educational needs.
- Update plans of care to reflect changes in condition or new information.
- Collect and document data from patients, families, and healthcare professionals.
- Patient Visits:
- Respond to patient and family needs, changes in condition, and new admissions.
- Provide interventions, treatments, and pain management to meet patient needs.
- Plan of Care:
- Develop, implement, and update plans of care in collaboration with interdisciplinary teams.
- Delegate visits and make referrals to appropriate disciplines and volunteers.
- Miscellaneous:
- Participate in meetings to plan and evaluate patient services.
- Precept new staff and students.
- ADN or BSN required.
- Current, unrestricted RN license in the state of employment.
- One year of nursing experience preferred.
- Valid driver's license, auto insurance, and access to a reliable vehicle.
- Ability to develop, maintain, and apply knowledge of hospice and end-of-life processes.
- Ability to maintain professional boundaries and confidentiality.
- Basic Microsoft Office skills and intermediate electronic health records skills.
The estimated salary for this position is $60,000 - $80,000 per year, depending on experience. Additionally, Agrace offers a comprehensive benefits package, including medical, dental, and vision insurance, 401(k) matching, and paid time off.
