Job Summary:

St Croix Hospice seeks a skilled Registered Nurse to lead our hospice care team. As a Nurse Care Manager, you will plan, organize, and direct hospice care, focusing on community health education and experience.

Key Responsibilities:

  • Conduct comprehensive assessments of patients and families to determine hospice needs.
  • Develop and implement individualized care plans that meet patient and family needs.
  • Collaborate with interdisciplinary teams to provide high-quality patient care.
  • Provide education and support to patients and families on palliative care and end-of-life options.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
  • Supervise and delegate responsibilities to hospice aides as needed.
  • Participate in quality assessment and performance improvement initiatives.

Requirements:

  • Graduate of an accredited school of nursing.
  • One to two years of recent acute care experience in an institutional setting, with home care experience preferred.
  • Current licensure in the state.
  • Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills; nursing skills per competency checklist.
