Job Summary:

The Oncology Nurse Navigator is a key member of the multidisciplinary team at Temple Health, responsible for providing expert oncology nursing practice to ensure seamless care coordination for cancer patients and their families.

Key Responsibilities:

  • Provide access to quality care for cancer patients and families through expert oncology nursing practice.
  • Collaborate with the multidisciplinary team to assist patients along the continuum of care from abnormal findings, diagnosis, and through survivorship.
  • Serve as a clinical resource with expertise in oncology care management.
  • Provide guidance to patients related to scheduling appointments, removing access-related barriers, including insurance and transportation, and answering clinical care-related questions.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare providers to ensure timely and effective care coordination.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.

Requirements:

  • Bachelor's Degree in Nursing (Required)
  • Masters Degree in Nursing (Preferred)
  • RN-LIC - PA Registered Nurse License (Required)
  • OCN - OCN - Oncology Cert Nurse (Preferred)
  • 5 Years experience in nursing with 2 years in oncology nursing (Required)
  • 1 Year experience as an oncology nurse case manager or patient navigator (Preferred)
