Oncology Care Coordinator

The Nurse Navigator plays a vital role in ensuring seamless care coordination for cancer patients and their families. This involves expert oncology nursing practice in providing access to quality care, addressing barriers to care, and facilitating communication among healthcare providers.
Key Responsibilities- Assist patients in navigating the healthcare system, including scheduling appointments and addressing insurance and transportation-related issues
- Provide education and guidance to patients on clinical care, treatment options, and available resources
- Collaborate with the multidisciplinary team to ensure comprehensive care and coordination
- Serve as a clinical resource and expert in oncology care management
- Bachelor's Degree in Nursing (Required)
- 5 Years of experience in nursing, with 2 years in oncology nursing (Required)
- 1 Year of experience as an oncology nurse case manager or patient navigator (Preferred)
- Oncology Certified Nurse (OCN) certification (Preferred)
