Social Services Coordinator

We are seeking a skilled and compassionate Social Worker to join our team at Tarrytown Rehabilitation & Nursing Center. As a Social Worker, you will play a vital role in providing high-quality social services to our residents, ensuring their physical, emotional, and social well-being.
Responsibilities:- Assist with planning and implementing comprehensive social services programs to support residents and their families.
- Coordinate admissions and discharges to ensure facility census is maintained at optimal levels.
- Participate in interdisciplinary team meetings to discuss resident care and services.
- Provide therapeutic interventions to help residents cope with social and psychological aspects of their illnesses or disabilities.
- Maintain accurate and up-to-date documentation in resident medical records.
- Communicate effectively with residents, families, medical staff, and other team members to ensure seamless care and services.
- Master's degree in Social Work (MSW) or related field.
- Minimum of 2 years' experience in social work or related field.
- Valid licensure as a Licensed Master's in Social Work (LMSW) or related field.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
