Social Services Coordinator

This position plays a vital role in ensuring exceptional customer service and quality of care at our hospital. As a key member of our interdisciplinary team, you will provide services to patients, families, and significant others, including biopsychosocial assessments, psychiatric diagnoses, treatment planning, and therapy sessions. Your expertise will help support and enhance biopsychosocial functioning, and you will work collaboratively with patients, families, and staff to identify and address their needs.
Key Responsibilities- Conduct biopsychosocial assessments and develop treatment plans to address patients' needs
- Provide individual, couple, family, and group therapy sessions to promote mental health and well-being
- Collaborate with interdisciplinary teams to develop and implement care plans
- Identify and connect patients with community resources to support their needs
- Participate in quality improvement initiatives to enhance patient care and satisfaction
To be successful in this role, you will need a Master's degree in Social Work, a Licensed Social Worker (LSW) certification, and social work licensure. You should also possess strong interview, assessment, and problem-solving skills, as well as the ability to work collaboratively with patients, families, and staff.
