Job Summary

We are seeking a highly skilled and compassionate Unlicensed Clinical Social Worker to join our team at Patient Centered Services LLC. As a key member of our interdisciplinary healthcare team, you will play a vital role in providing psychosocial care and support to patients and their families in home health and community settings.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patients' needs and develop personalized care plans.
  • Collaborate with licensed social workers, nurses, physicians, and therapists to ensure holistic patient care and address mental, social, and emotional needs.
  • Provide case management services, including coordination with community resources and support programs, to ensure patients receive necessary assistance.
  • Document patient interactions and maintain accurate records in our electronic medical record (EMR) system.
  • Facilitate access to social services and community resources, promoting patient independence and well-being.
Requirements
  • Bachelor's degree in Social Work (BSW) or related human services field from an accredited institution.
  • Minimum of 1-2 years of experience in a social work or human services role, preferably in healthcare or home health settings.
  • Strong understanding of social work principles, effective communication skills, and ability to manage multiple cases and tasks with strong organizational skills.
Preferred Qualifications
  • Masters degree in Social Work (MSW) from an accredited program (in progress or pending licensure).
  • Experience working with diverse populations, such as pediatric or geriatric patients.
  • Multilingual abilities, especially English/Spanish, are preferred but not required.
